New Patient Information

Have your records transferred from your previous office. If you have records and x-rays to be transferred from another office, please have those sent to us. See this document for where records are to be sent. Your previous provider may require you to complete their records release form, so please check with them. 

If x-rays are not received prior to your appointment, x-rays will be taken at your first appointment.

Once we have recieved your records and new patient paperwork, our front office team will reach out by email or phone to get you scheduled for your new patient appointment. All New Patients are seen by Dr. Engelmann at their first visit and she will work with you to determine the right care for you.
